Requirements:

Associates degree required, BSN preferred

HOME HEALTH AID JOB DESCRIPTION

Position type available is Full-time or Part-time

Work at the direction of the Registered Nurse according to the scheduled duties and complies with Bowes In Home Care (BIHC)’s policies and procedures and all applicable federal, state, local and Medicare laws and regulations. Responsible in assisting patients in performing daily living activities, such as bathing, dressing, toileting, grooming, ambulation and feeding. Observe changes in resident’s condition, report findings to the registered nurse or licensed therapist and complete all necessary documentation timely and according to BIHC standard. Ensures all service provided is completed according to BIHC standards and expectations.

Requirements:

Minimum 1 year experience in Home Health.
Completion of a nursing assistant training program here in Illinois or as prescribed by IDPH.
Copy of Certification or Licensure if applicable.
Copy of CPR certification.
Must complete and pass the competency examination conducted by a registered nurse.
Be able to travel to do patient care.
Accessible 24 hours a day, 7 days a week.

Position Summary:

Provide care for the following activities of daily living for patients using appropriate and safe techniques in performing personal care such as bathing, grooming, toileting, feeding, medication reminders, and dressing.
Assist with transfers on patients with sufficient balance to do stand pivot transfers or may use adaptive and safety equipment when available and applicable.
Provide assistance in ambulation and exercise of the patient under the direct supervision of a therapist or by an Agency licensed to provide home health or home nursing services.
May be responsible for emptying an ostomy bag and providing assistance with other patient-directed ostomy care only when there is no need for skilled skin care or for observation or reporting to a nurse.
May assist in and perform mouth care, which may include denture care and basic oral hygiene, including oral suctioning for mouth care.
Observe residents for any changes in condition and report to the supervising Case Manager: registered nurse or licensed therapist.
Maintain personal daily time records according to Agency guidelines.
Participate in case conferences, meetings, In Services and training programs as required by the Agency to ensure professional growth.

OCCUPATIONAL THERAPIST JOB DESCRIPTION

As an Occupational Therapist, you will visit multiple patient’s homes each day and provide them with a full range of OT care.

Position Requirements:

Minimum 1-3 years of experience in Occupational Therapy and heath care environment
Current Illinois OT license
Valid driver’s license

Position Overview:

Provide assistance, guidance and instructions to the patient, family, caregiver and home health aide with the use of therapeutic, creative and self-care activities.
Review referral and patient’s clinical record to determine need for OT services.
Participate in the development, implementation, review and revision of the plan of treatment.
Document clinical and progress notes and summaries.
Conduct initial Occupational Therapist evaluation, assessment and reevaluation of the patient’s level of function.
Prepare treatment plans.
Teach other personnel any exercises, techniques and added instructions with patient’s care.
Conduct on-site supervision every four to six visits.

MEDICAL SOCIAL WORKER JOB DESCRIPTION

Position Summary:

Licensed Medical Social Worker (MSW) ensures Bowes In Home Care (BIHC) is compliant with all applicable federal, state, local and Medicare laws and regulations in providing significant information about the resources and support available in the community for the patient. Provide psychotherapy, counseling and other psychological approaches to help a patient to expand and strengthen their network of social support in accordance to the skill set of a Medical Social Worker. Ensures all service provided is completed according to BIHC standards and expectations.

Position Requirements:

Must be a licensed MSW
Minimum of 1 year social work experience
Minimum of 1 year home health experience

Performance Standards:

Compliant with all applicable federal, state, local and Medicare laws and regulations.
Provide referral to community resources to assist patient and family.
Participate in the development, implementation, review and revision of the plan of treatment as a member of the home health team.
Provide consultation in social/psychological issues to other team members.
Be a resource and support person for the patient, family, aides and BIHC in regards to care and services provided.
Participate in case conference, meetings, In Services and training programs as required to ensure professional growth.

SPEECH LANGUAGE PATHOLOGIST JOB DESCRIPTION

Position type available is PRN.

Position Summary:

Speech Language Pathologist

Compliant with all applicable federal, state, local and Medicare laws in providing patient care and implementing treatment of care with speech, language and communication disorders. Responsible in conducting necessary evaluation, treatment, assessment, follow-up and reevaluation. Ensure that all necessary documents are completed and all service provided is completed according to BIHC standards and expectations.

Position Requirements:

At least 1-3 years of experience working as a Speech Therapist in healthcare.
Must be Illinois state licensed and have a PEL (professional educator’s license).
Valid Driver’s license and Insurance.
Be able to travel to do patient care.

Performance Standards:

Review referral and patient’s clinical record (when available) to determine the need for speech therapy services.
Conduct initial evaluation, assessment, follow-up and reevaluation of patients with speech, hearing or language disorders and establish a plan of treatment.
Providing rehabilitative services.
Providing instructions and coordinate with other health care team, patient and family members with the home rehabilitation program.
Providing written instructions when applicable to ensure continuum of care.
Makes supervisory visits to the patient no less than two weeks to ensure proper care and services were given.
